Needs help to process data from a form in php

hello there,

under "about us", contact page collect data. not processed.
i used new php. anyhow, want process send data collected email address, , @ same time, display simple thank page. attaching code.
i hope can me out have been trying quite while , need done asap...

avrdude: verification error, first mismatch at byte 0x0000 0x0c != 0x62
